Report a Concern

Your leader is a good place to start if you have questions, but if you are uncomfortable bringing your concerns to your leader or to other internal resources, you may choose to submit a CareSmart report.

CareSmart is operated by a third-party provider and allows you to report concerns securely and confidentially. Additionally, you always have the option to submit your concern anonymously. Please be open and honest when making a report and provide as much detailed information as possible to help our Company conduct a full and proper investigation.

Use one of the below options to submit a CareSmart report.

When you submit the report, you will be issued a Report Key. Please write it down and keep it in a safe place, because it cannot be retrieved later. You may use the Report Key, along with the password of your choosing, to return to CareSmart through NAVEX website or telephone hotline to check on your report or to provide a Follow-up report.

Once submitted your report will be logged and routed to someone designated by PetSmart for follow up. Please note, if your report mentions a specific individual within PetSmart, it will not be made available to that individual.
