Chautauqua’s policies and standards contain expectations for conducting business with the highest standards. As such, we are committed to a culture of honest and open communication at every level. Employees should feel comfortable in approaching management and Human Resources in any instance where they believe violations of policies or standards have occurred.
We all share in an important responsibility to report these concerns, as prevention or remedies cannot occur unless the organization is aware of a problem or risk.
Any employee or other person who has been subjected to, witness of, or otherwise aware of violations of law, policies, or standards is encouraged to report (verbally or in writing) to management, Human Resources, or with Ethics Point at chq.ethicspoint.com or by toll free phone at (855) 673-1087. Management is required to report and take the appropriate steps to address reports received in conjunction with Human Resources.
If an employee makes a report to management and believes that the manager is not taking appropriate action, the employee should report this inaction to Human Resources, another supervisor or manager, or Ethics Point. If an employee believes that a supervisor violated this policy, then the employee should report the matter to Human Resources, another supervisor or manager, or Ethics Point.
In situations where you prefer to place an anonymous report, you are encouraged to use this website or phone line, hosted by EthicsPoint, a third-party service provider. See the EthicsPoint FAQs for more information.
Upon receipt of a report, the Institution will take appropriate steps to investigate the report. This may include interviews of appropriate individuals (i.e., the person reporting, the accused, possible witnesses, etc.). The Institution will take action based on its assessment of the situation, which may include corrective action. Disciplinary action may be taken as appropriate and may not necessarily be administered in a progressive manner.
Confidentiality will be protected to the greatest extent possible, consistent with conducting a full investigation. However, the Institution cannot guarantee complete confidentiality.
The Institution strictly prohibits and will not tolerate retaliation against anyone who in good faith makes a report participates in an investigation of a report. Individuals who become aware of anyone engaging in retaliation prohibited by this policy should immediately report the matter to a supervisor, manager, Human Resources, or Ethics Point. Any person who engages in retaliatory conduct prohibited by this policy will be subject to appropriate disciplinary action, up to and including termination of employment.

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.
