What should I do if I lose my report key or password?

Because of the high level of security that is maintained for these reports, if you lose your report key or password, you will need to fill out a new report. You should mention in the new report that it is related to a report you previously filed.

Please note that once a matter is closed, this follow up feature will be available for 7 days following the close date. If you are unable to follow up on a closed matter, and you have new information to provide, please submit a new report and reference the previous report in the details of your concern.
