To ensure that security and confidentiality is maintained, if you lose your report key or password, you will need to file a new report. When you file a new report, it is important to inform us that this concern is related to a report or question you previously submitted.
What if I remember something important about the incident after I have filed a report?
To provide additional information pertaining to your concern, you can either call 1-888-774-9100 or go to kp.org/compliancehotline with your report key and password.
How can you tell if an issue or problem is appropriate for the KP Ethics and Compliance Hotline?
Ask yourself the following questions before contacting the KP Compliance Hotline. If you answer “yes” to either, we recommend that you report your concern by either calling the 1-888-774-9100 or going to kp.org/compliancehotline immediately.
- Is KP at immediate risk if this behavior, action, or activity continues?
- Is the behavior, action, or activity potentially illegal, prohibited, or not compliant with regulations or KP policy?

If you are a member or patient with general questions about benefits, co-pays, medical referrals, etcetera, please call the customer service number located on the back of your membership card.
If you are an employee, you may also report your concern to your supervisor, manager or Compliance Officer.
