The UCLA Department of Intercollegiate Athletics (DIA) takes violations of University and DIA policies, NCAA bylaws, and federal and state laws seriously. We have selected EthicsPoint to provide a confidential means for reporting suspected misconduct or concerns. You may report your concerns online or by phone. With either method you can choose to remain anonymous. At the end of your report, you will create a PASSWORD and receive a REPORT KEY, to allow you to continue to communicate with us online or by phone. Please save your REPORT KEY and PASSWORD. Your PASSWORD cannot be reset or recovered. Please note that if you do not retain your REPORT KEY and PASSWORD, you will not be able to view the status of your report, submit additional information, or respond to follow-up questions.
All reports submitted through EthicsPoint will be reviewed by the UCLA Dean of Students Office, University Compliance (outside of Athletics) and the Faculty Athletic Representative. Reports may also be referred to other university entities (such as the Title Office) or back to the DIA as needed.
This service is not intended to replace the opportunity to report issues or concerns in person. You may always report your concerns directly to coaches, other DIA employees or to University officials. Note that personnel issues may also be addressed to Human Resources or Labor Relations.
EthicsPoint is NOT a 911 or Emergency Service:
Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please contact your local authorities.
