After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

We encourage open and honest communication, making it the expectation, not the exception. If you believe that there has been a violation of a policy or standard, feel free to approach your supervisor, Compliance, Human Resources or anyone in a leadership position.
We understand that there may be times where you are uncomfortable approaching someone or you prefer to report an issue anonymously. In such situations we recommend using the CHS Helpline hosted by a third-party provider, EthicsPoint. The information you provide to EthicsPoint will be sent to CHS confidentially, and access is limited. And if you make an anonymous report, your information won’t be shared with CHS.
