TO MAKE A REPORT ONLINE

If the situation involves an employee actively working in the operation (pilot, mechanic, flight attendant, cargo agent, ACS agent/Elite Service) under the influence of drugs or alcohol, please call. Written reports can be delayed.

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 2-4 business days, use your report key and password to check your report for feedback or questions.
