HOW TO ACCESS YOUR SUBMITTED REPORT TO LEARN THE STATUS OR PROVIDE ADDITIONAL INFORMATION

When you submit an Anonymous Report you will be issued a Report Key and asked to create a Password. It is very important that you keep the Report Key and Password as it is the ONLY way you will be able to access your submitted report to obtain a status, answer the investigator’s questions, or provide additional information. If during the course of the investigation we require additional information, we will post a question and it will appear when you log in. At that time, you will be able to provide a response as well as any additional information you may have and continue the anonymous dialog.

We encourage you to check back weekly to see if the status has been updated or if additional information is needed. Thank you.
