To Make a Report

You may use either of the following two methods to submit a report:

  • Select the "Make a Report" link at the top of this web page.
  • OR

After you complete your report you will be assigned a unique code called a "report key." Write down your report key and password and keep them in a safe place. After 5-6 business days, use your report key and password to check your report for feedback or questions.

Further, our Standards of Conduct contain guidelines for proper workplace conduct, acceptable use of Company systems, and more to ensure as a business we act responsibly and with the highest standards of ethics.

Appspace is committed to an environment where open, honest communications are the expectation, not the exception. We want you to feel comfortable in approaching Human Resources, your Manager, Department Head, or any member of the Executive Team in instances where you believe violations of policies or standards have occurred.

However, we recognize there could arise a situation where you may feel more comfortable reporting a concern confidentially. If/when this should occur you are encouraged to use this site, hosted by a third-party hotline provider, EthicsPoint, to submit a report either electronically or by phone. You are encouraged to report violations as covered in our Standards of Conduct, Employee Handbook, or any other concern you deem inappropriate and important.

The information you provide – whether via phone through the hotline or electronically using the web intake form - will be sent to the Appspace HR department by EthicsPoint on a confidential and anonymous basis. However, if you should choose and/or wish to be contacted to discuss your concern you may disclose your contact information. It is your decision whether to disclose your contact information. Either way your concerns will be reviewed promptly, completely, and with the utmost professionalism.
