Vendor/Supplier 3rd Party Due Diligence Form

Submit a Covered Business Partner (CBP) for review through the Office of Compliance screening process. A CBP can be a supplier, vendor, agent, subcontractor or other business partner that provides goods or services on behalf of Cencora anywhere in the world.

Managers and above use this form to capture any report, concern or potential violation of our Code, Company policy or the law.

Advisory Engagement

Obtain Office of Compliance review, guidance and approval of advisory engagements, such as advisory boards, surveys, focus groups, etc.

Business Gratuities & Sponsorships

Request Office of Compliance review, guidance and approval of business gratuities, items of value, life event gifts, educational items, entertainment, charitable or commercial sponsorships to or from customers, health care professionals, government officials, potential referral sources, vendors or suppliers.

Privacy Incident Report Form

Use this form to report any event or suspected event in which Personal Data or Personal Information has been disclosed to an unauthorized party, accessed by an unauthorized party, or used in an unauthorized manner, regardless of whether the disclosure or use was intentional.

Field Services Rules of Engagement Incident Report

Field Services Team submits any potential incidents where a program or client’s Rules of Engagement were not adhered to.
